Before You Begin

Once your registration has been received, you'll be asked to complete a brief pre-program wellness survey. You will receive this within one hour of submitting your registration.

This survey helps us better understand participants' overall health and wellness before the program begins and allows us to measure the program's impact over time.

The survey may include questions about:

  • Physical activity

  • Nutrition habits

  • Stress and overall well-being

  • General health and lifestyle

Your responses are kept strictly confidential and will only be used for program planning, evaluation, and reporting purposes. Information shared with our funders or partners will only be presented in aggregate form, meaning no individual participant will ever be identified.

Participants will also be invited to complete a post-program survey at the end of the program to help us evaluate outcomes and continue improving our services.
